Step 3 :-
After signing up on this blog, you can easily create any type of form. Here you can create slide type or single page. see the image below and just click on left side and you can see it will automatically paste on right side.
After creating your form , just save this from bellow "save form" button option. and click on "code" and copy this code. at last, paste it on your blog.
